Global Home Care Services also offers discharge service and assistance. This program assists the client and their family with discharge from:
- Hospital
- Rehab
- Emergency Room and Ambulatory Surgery
With this service, we provide escort to the client’s home, medication pick up, meal preparation, environmental adjustments, and general client stabilization. Services are available from a minimum of four hours to 24/7 supportive care. In many ways, this service helps prevent re-hospitalization or re-admission. It also fills the gaps in the client’s transition to full recovery.
